You are a Block Vision Group Member if:

• You have an ID card with the Block Vision logo displayed on the front side in the upper left corner.
• You receive your vision care coverage through your place of employment or are a covered dependent of a member who receives vision plan coverage through their place of employment.

You are a Health Plan Member if:

• You receive your vision coverage through your health plan.
• Members enrolled through Block Vision's client health plans use their health plan ID card to access vision coverage. You do not receive a Block Vision ID card.
